Scrotal aesthetics · Potts Point, Sydney
A lower, fuller hang. Relaxed, not retracted.
The scrotum has muscle in it. In the cold, in water, with nerves or during exercise, that muscle pulls everything up tight. Scrotal lowering is an in-clinic treatment that relaxes it for a few months, so the scrotum hangs lower and smoother, looks fuller, and stays that way in cold air and water. Some men also find it more pleasant to the touch, and say they feel more confident. At Male by Cleon in Potts Point, it’s planned in a private, doctor-led consultation, with an honest word about what it can and can’t do.
This treatment uses prescription medicine prescribed off-label, which Australian law doesn’t allow us to advertise to the public. It’s discussed with you during a consultation, and offered only where it’s clinically appropriate. Outcomes vary between individuals.
How it works
The scrotum isn’t just skin. There’s a thin sheet of muscle in its wall, and it’s that muscle, not the skin, that decides how high and tight everything sits.
Scrotal lowering, also called scrotal relaxation, is an in-clinic treatment that relaxes the thin muscle in the scrotal wall, the dartos, which wrinkles the skin and draws the scrotum up in the cold. Relaxed, the scrotum hangs lower, looks fuller and smoother, and stays down in cold air and water. The effect builds over one to two weeks and lasts about three to four months. It uses prescription medicine prescribed off-label, so it’s discussed in a private, doctor-led consultation at Male by Cleon in Potts Point, Sydney: $100 in clinic, or a free video call first. Some men say it’s more pleasant to the touch and that they feel more confident; there are no published trials measuring either. It doesn’t change the size of the testicles or the penis.
Just under the skin sits the dartos, a thin layer of muscle that wrinkles the skin and draws the scrotum up. Deeper, the cremaster lifts the testicles themselves.
Cold air and cold water, nerves, exercise and arousal all make it contract. That’s why the scrotum shrinks and bunches in the pool, and hangs loose in a warm shower.
A small dose, placed in the skin, quietens the dartos for three to four months. The skin smooths, the scrotum hangs lower and looks fuller, and cold has far less effect. It also calms the sweat glands there.
What men ask for
Six things men come in for. We’ll tell you which are likely for you, and which we can’t promise.
There are no published trials of this treatment for appearance, so what follows is what it does to the muscle, and what men tell us. Every plan starts with a check of the testes and the skin, and treatment is performed by our clinical team under medical supervision.
With the wall relaxed, the scrotum drops and hangs more freely. The change is in how it sits, not in the size of the testicles.
Lower and looser reads as fuller, in underwear and in swimwear. Nothing is added. It’s the same anatomy, no longer held up tight.
Cold air and water have far less effect, so it doesn’t shrink right up in the pool, the surf or a cold change room. The deeper cremaster can still lift the testicles a little.
The dartos is what wrinkles and bunches the skin. Relaxed, the skin sits smoother and flatter for as long as the effect lasts.
The same treatment quietens the sweat glands in the skin, so many men notice less scrotal sweating and chafing. For heavy sweating elsewhere, see hyperhidrosis.
Some men say relaxed skin is more pleasant to the touch, and that they feel more confident. No trial has measured either, so we say may, not will. It doesn’t change erections, orgasm or ejaculation.
It won’t make the testicles or the penis bigger, and it won’t lift a scrotum that hangs lower than you’d like; that’s surgery, and we refer for it. The effect wears off in three to four months. If it hangs lower than you expected, that wears off too.
Lowering or lifting?
No. They pull in opposite directions, and men mix them up. Here’s the difference, so you know what to ask for before you book.
| At a glance | Scrotal lowering, in clinic | Scrotal lift, surgery |
|---|---|---|
| What it does | Relaxes the scrotal wall, so it hangs lower, smoother and fuller-looking. | Removes excess skin, so the scrotum sits higher and tighter. |
| Who asks for it | Men whose scrotum sits high and tight, especially in the cold. | Men whose scrotum hangs lower than they’d like, often with age. |
| How it’s done | In clinic, after numbing cream, in about 30 minutes. | Under anaesthetic, by a plastic surgeon or urologist. We refer. |
| Downtime | None. Sex and hard exercise wait a day or two. | A couple of weeks, with stitches and swelling. |
| How long it lasts | About three to four months, then it’s repeated if you want to keep it. | Permanent, though the skin keeps ageing. |
| If you change your mind | It wears off. | It doesn’t. |
Not sure which you’re after? Say so. The consultation is where it gets worked out, with an examination and an honest answer.
Intimate skin
Even fewer men mention this one. Skin tags, itching, darker skin, haemorrhoids and small tears are all common, and most are simple to sort out once someone actually looks.
Soft, painless flaps of skin around the anus, often left behind by an old haemorrhoid. They’re harmless. If they bother you or make cleaning harder, removal can be discussed.
Usually there’s a cause: moisture, over-cleaning, scented soaps or wipes, a skin condition, haemorrhoids, and occasionally threadworms or an infection. Find the cause and it usually settles.
Darker skin around the anus is normal, and it often deepens with age and friction. Some men still ask about it, and we’ll be honest about what’s worth doing.
Swollen veins that can bleed, itch or bulge. Many settle with fibre, fluids and not straining. Others need treatment, or a referral.
A small tear that makes bowel motions sharply painful, often with a streak of bright red blood. Most heal with softer stools and treatment; some need a specialist. More on fissures.
Blood in your stool, a change in bowel habit or a new lump is always checked, with a referral if it’s needed. Heavy bleeding or severe pain needs same-day care.
We’ll look, tell you what it is, and either sort it here or send you to the right specialist, usually a colorectal surgeon. Every examination is done with your consent, in your own appointment time.
Worth a minute a month
Getting to know what’s normal for you makes a change easier to notice. It takes a minute, and the shower is the easiest place to do it.
Sudden, severe pain or swelling in a testicle is an emergency. Call 000 or go to the nearest emergency department.
Is this right for me?

Frequently asked
Scrotal lowering is part of Enhancement at Male by Cleon, alongside penile enhancement, erection enhancement, premature ejaculation and anal sphincter relaxation. Anything else, ask at your consultation or email hello@cleonclinic.com.
An in-clinic treatment that relaxes the thin muscle in the scrotal wall for about three to four months, so the scrotum hangs lower, looks fuller and smoother, and stays down in cold air and water. It uses prescription medicine prescribed off-label, so it’s discussed in a private, doctor-led consultation. It’s sometimes called scrotal relaxation.
Yes. Scrotal aesthetics, sometimes called scrotal rejuvenation, is the broader term for treatment of how the scrotum looks and feels, and scrotal lowering is the main treatment under it. Sweating, skin changes and the skin around the anus are covered on this page too.
Just under the skin sits the dartos, a thin layer of muscle that wrinkles the skin and draws the scrotum up. The treatment quietens that muscle, so the scrotum drops, the skin smooths and the whole thing hangs more freely. Nothing is added and nothing is cut.
Much less. The muscle that bunches the skin and draws the scrotum up is relaxed, so cold air and cold water have far less effect. The deeper cremaster muscle, which lifts the testicles themselves, isn’t treated, so a little lift in the cold is still normal.
It can look fuller. A scrotum that hangs lower and looser reads as fuller in underwear and swimwear, because it’s no longer held up tight against the body. The size of the testicles and the penis doesn’t change.
Yes, for as long as the effect lasts. The dartos is what wrinkles and bunches scrotal skin, so once it’s relaxed the skin sits smoother and flatter.
Often, yes. The same treatment quietens the sweat glands in the skin it’s placed in, so many men notice less scrotal sweating and chafing. For heavy sweating elsewhere, there’s more on our hyperhidrosis page.
Some men say relaxed, smoother skin is more pleasant to the touch, for them and for a partner. No trial has measured it, so we say it may, not that it will. It doesn’t change erections, orgasm or ejaculation, and it isn’t a treatment for erection problems; that’s covered on our erection enhancement page.
Some men tell us it does: in the change room, at the beach, or in bed. We can’t promise how you’ll feel. We can tell you exactly what the treatment does, and be honest about whether it’s likely to give you what you’re after.
A prescription medicine, prescribed off-label. Australian law doesn’t allow prescription-only medicines to be advertised to the public, so we can’t name it here. We’ll go through it with you during your consultation: what it is, why we’d use it, its risks, and whether it suits you.
That a medicine is prescribed for a use other than the one it’s registered for. Off-label prescribing is legal and common in medicine, when a doctor judges it appropriate and you’ve agreed to it after hearing the benefits, the risks and the alternatives. We’ll talk it through with you at your consultation.
Numbing cream first, then a small dose is placed at several points in the skin of the scrotum. It takes about 30 minutes. Most men describe brief, mild stinging rather than pain, and some tenderness or a bruise for a day or two afterwards.
The scrotum starts to relax within a few days, with the full effect at one to two weeks. It lasts about three to four months, then wears off gradually. If you want to keep it, it’s repeated.
None. You can go back to work straight away. Give it a day or two before sex, hard exercise, hot baths and saunas, and avoid rubbing the area on the day. Cyclists may prefer a few days off the saddle.
It’s a small treatment, but not risk-free. Bruising, tenderness and swelling for a few days are the most common. The scrotum can hang lower than you’d like, or less than you hoped, or slightly unevenly, until the effect wears off. Infection is rare. Every risk is explained before you decide.
The treatment is placed in the skin of the scrotum, not in the testicles. There’s no evidence that it affects sperm, testosterone or fertility, but it hasn’t been formally studied for that either. If you’re trying for a baby, tell us and we’ll talk it through before anything is planned.
No. It does the opposite: it lets the scrotum hang lower. If yours hangs lower than you’d like, a lasting change means surgery, a scrotal lift, and we’ll refer you to a plastic surgeon or urologist if that’s what you want.
Often, yes. If the examination is clear, you’ve heard the risks and you’re sure, it can be done in the same visit, and the $100 consultation fee comes off it. If you’d rather think it over, it’s booked for a later appointment instead.
A face-to-face consultation at Male by Cleon is $100, which comes off your treatment if it’s done on the day, and a private video call is free. Because the treatment uses prescription medicine prescribed off-label, it’s priced in consultation, and you’ll know the full cost before anything starts. Cosmetic treatments aren’t covered by Medicare or private health insurance.
Yes. In most men one testicle, usually the left, sits lower, and one is often a little larger. That’s normal. A new change in one testicle, such as a lump, swelling, heaviness or an ache, isn’t, and should be checked promptly.
Once a month, after a warm shower, roll each testicle gently between your thumb and fingers and feel for a lump, swelling, hardness or a change in size. It’s normal for one to be slightly larger or lower, and to feel a soft tube at the back. See a doctor promptly about any change.
Yes, with your consent. An examination of the testes and skin is part of every scrotal consultation, because a few things need ruling out before anything cosmetic.
Yes. We assess them, treat many of them in clinic, and refer to a colorectal surgeon when a procedure or a specialist opinion is needed. For a fissure that hasn’t healed, relaxing the sphincter in clinic is one option; there’s more on our anal sphincter relaxation page. Bleeding from the bottom should always be checked.
Male by Cleon consultations run in their own appointment times, with space either side, and there’s no shared waiting area. Clinical records are kept under full medical confidentiality, and nothing is shared outside the clinical relationship without your consent.
Male by Cleon is at Ground 02, 46a Macleay Street, Potts Point, a short walk from Kings Cross station and close to Elizabeth Bay, Darlinghurst, Rushcutters Bay, Woolloomooloo and the CBD. If you’re further away, start with a free video call.
